If the death took place at home:
First call the physician in charge of the deceased patient or the ambulance physician who issues a death report or a statement of death. The statement of death is the basic document confirming the cause of death and is required to obtain SHORT DEATH CERTIFICATES (in the Civil Registry Office).
After the said document is obtained, the deceased person should be transported to the cold room by the funeral company. In such a case, just call JASEK Funeral Company at 24/7 phone number +48 32 231 13 47 where the employees receive your order and arrange coming to pick the deceased one up.
We transport the deceased person to our own cold room where s/he stays until the funeral.
If the death took place in public space:

If the death took place in an accident:
If the death took place in an accident or there is a justified suspicion that it can result from a crime, you will need not only the statement of death but also the prosecutor's permission to bury the body. The bodies of people who died or were killed in public spaces are transported, on request of the competent authority (most usually a prosecutor), to the Forensic Medicine Centre to determine the cause of death.
If the death took place in hospital:
The doctor in charge issues the statement of death which you should take to Jasek Funeral Company. All the other formalities and tasks related to funeral organisation are taken over by us.
